Children & Family Well-Being Service

Key workers deliver intensive support to families underpinned by an assessment (CAF).  A key worker will meet with your family weekly and offer support for a range of concerns or issues your family is facing through outreach, normally in your home for a period of 12 weeks with an agreed action plan made between the keyworker and the family around what changes the family are seeking to change together with help.

Referrals can be made as self-referral or through education.
